Renal Oxygen Consumption, Insulin Sensitivity, and Daily Caloric Restriction in ADPKD (EXPLORE)

Overview 

The purpose of this pilot study is to assess kidney oxidative metabolism and insulin sensitivity after a 2-year weight loss intervention in those with autosomal dominant polycystic kidney disease (ADPKD) who are overweight or obese. 

Eligibility Criteria 

  • Ages 18 – 65
  • ADPKD diagnosis based on the modified Pei-Ravine criteria
  • Body-mass index of 25-45 kg/m^2 
  • Estimated glomerular filtration rate (eGFR) greater than or equal to 30 mL/min/1.73 m^2. 
  • Total kidney volume (htTKV) > 600 mL
  • No plans for extended travel (>2 weeks) without internet access during the 12-month intensive period

Additional inclusion/exclusion criteria will apply. 

Participation

Participants will be randomly placed in either the daily caloric restriction group or the standard advice control group. The daily caloric restriction group will participate in a 2-year, group-based, behavioral weight loss intervention based on a 30% reduction in caloric intake and increased physical activity. The standard advice control group will receive an initial consultation with a registered dietician regarding current clinical recommendations for ADPKD without subsequent counseling sessions. 

Renal oxygen consumption (the amount of oxygen that the kidneys consume) and insulin sensitivity will be assessed at baseline and at 24-months.
